Post-Hospital Care

After a hospital stay, caring for an elderly loved one can be very challenging. Especial if they have special needs that you may note be trained on. Comfort Keepers offers tailor-made care plans that include a variety of hospital follow-up options, such as personal care, dressing changing, assisted living and home care. We facilitate the process of uplifting care in the home and a number of other services.

The period immediately following discharge from the hospital is the critical period of recovery. Rehabilitation after injury or illness can be a complicated process, especially for seniors.

In addition to physical challenges, depression and anxiety can occur, caused by social isolation and changes in normal routines. Nurses trained in infection control and elderly care can help the recovering stay home safely and reduce the risk of re-injury after hospitalization by following a discharge plan. It can also provide respite and a short period of relief for family members and other carers. Collaboration Care

 Comfort Keepers often works cooperatively with other home health services so that seniors and other adults can receive a full range of care that helps speed up the recovery process. The Caregivers from Comfort Keepers make transitions from hospital to home as safe, and seamless as possible.

Caregivers can pick up prescriptions or other supplies at the hospital, or even at a local pharmacy or drugstore. Patients can be reminded to take their medicines on time. Your caregiver can help with transportation to scheduled follow-up appointments.

Encouragement and recovery are hard work, and it is not uncommon for patients recovering from surgery to feel melancholy or get frustrated. Comfort Keepers is there to encourage and support the doctor-prescribed physiotherapy and speech therapy. In addition to providing community and meaningful activities.
